We are excited to offer the following In House Membership plan to patients who DO NOT have dental insurance.

Who is Eligible?

This In-Office membership plan is designed for patients who do not have dental insurance.

Easy Monthly Payments!

Standard Membership

• Adult:  $35/month*

• Children:  $30/month

*Children under 19 are eligible under child membership

​

Periodontal Membership

Some patients require additional visits during a calendar year to maintain healthy gums and bone.  Membership level determined at initial exam.

3 hygiene visits per year--$50/month

​

Membership Benefits

• 2 cleanings per year (standard membership only, periodontal membership may be necessary for some individual health needs)

• 2 Examinations per year

• 1 set of bitewing x-rays

• 1 emergency visit per year during normal business hours ( includes one PA x-ray)

• In office fluoride treatments ( Children only, 2 treatments per year)

• 20% off all dental treatment completed with 12 months of enrollment if paid in full at time of service (includes any additional x-rays)

• This includes elective cosmetic procedures like teeth whitening, crowns, and veneers.

“The Fine Print”

• It is only good when used with Eric S. Powell, DDS. Therefore, if you are referred to a specialist, they will NOT offer this discount.

• It CANNOT be combined with any other dental insurance or discount.

• This is a dental discount plan and is NOT dental insurance.

• Dental treatment needed following any type of injury where a lawsuit is filed and therefore outside medical, auto, disability, or workman’s comp type insurances are involved, this discount plan CANNOT be used.

• Non-Refundable; no refunds are given if patient chooses not to use their dental membership.

• Plan is subject to change yearly.

• All payments are due at time of service to receive the discount.  Any services not paid for at time of service will be billed at the usual, higher fee.

• Plan fees are valid only when paid at time of enrollment.

• Botox and Restylane are not included in discount plan.

*If paying with cash or check the membership has to be paid in full for the year

*If you choose to extend your payment through Care Credit, the treatment discount is reduced to 10% due to merchant fees

Warranty

Dr. Powell Stands by his work and provides a warranty for crowns and bridge work for 5 years and fillings are warrantied for 2 years as long as you complete recommended dental treatments and let us monitor the work we have done twice a year during your dental check-ups and cleanings.
